A form, available by clicking here, [link not yet live] will need to be completed by anyone with parental responsibility should a child require medication during the school day. No medication will be administered without completion of this paperwork.
There will also be this data capture form [link not yet live] that we will ask parents/guardians to complete at time of parent interview in October. This is to ensure we are fully informed of any medical needs
